The height of the forehead and the position of the hairline are primarily genetically determined. Over the course of a lifetime, hormonal changes or external influences can also cause the hairline to recede further. Many people affected find a very high forehead disturbing and would like to have a forehead reduction or hairline correction to reduce the visible forehead area and move the hairline forward.

Whether a forehead appears “too high” is perceived differently in different cultures. The division of the face into three equal thirds often serves as a guide: the distance from the hairline to the bridge of the nose should be approximately the same as the distance from the bridge of the nose to the tip of the nose and from the tip of the nose to the chin. If these proportions deviate, an aesthetic forehead correction may be advisable. As can be seen in the illustration of a medically indicated advancement of the hairline with lifting of the eyebrows (from a publication by our colleague Berenguer), this division changes the facial proportions to the patient's advantage. This procedure also raises the eyebrows if they have drooped.

How is a forehead reduction performed?

During forehead reduction surgery, the hairline is moved forward in a fine, arched incision. This shortens the forehead by up to 1–3 cm. At the same time, the eyebrows can be shaped to look more feminine if desired.

The operation takes about 1.5–2 hours and is performed under general anesthesia. Postoperatively, the sutures are very discreet; after a few months, the scar is barely noticeable.
